Information We Collect
Through the Service we collect “Non-Personal Information” and “Personal Information.” Non-Personal Information includes information that cannot be used to personally identify you, such as your IP address, information about your device, anonymous usage data, general demographic information we may collect, preferences you submit and preferences that are generated based on the data you submit. Personal Information includes information that can be linked to a specific individual, such as a name, contact preferences, address, phone number, email address, login information for third party sites and other information which you submit to us through the Service.
We collect personal information that you voluntarily provide to us when you register on the Services, express an interest in obtaining information about us or our products and Services, when you participate in activities on the Services, or otherwise when you contact us.
Information You Provide Us
The personal information that we collect depends on the context of your interactions with us and the Services, the choices you make, and the products and features you use. When you create an Account using the Services, we will ask you to provide us with your phone number. When using the Services you can elect to provide us with your email address, first name, last name, address, your phone number. We do not process sensitive information.
We may collect data necessary to process your payment if you choose to make purchases, such as your payment instrument number, and the security code associated with your payment instrument. We do not directly process or store your payment information. All transactions are securely handled by our third-party payment providers. These providers may collect and store your billing information in accordance with their own privacy policies. Information Collected Automatically from Your Use of the Service
Some information — such as your Internet Protocol (IP) address and/or browser and device characteristics — is collected automatically when you visit our Services.

Log and Usage Data. Referral Source Data. Information about how users arrived at the site (e.g., via social media, email campaigns, partner links) to analyze traffic sources and campaign performance.

Personal Information
Personal Information you provide to us is used to help us communicate with you and to provide you with the Service. In most cases, this means we will retain your personal data for the duration of your account’s active use. When we no longer have a legitimate business reason to process your information, we will delete or anonymize it. If immediate deletion isn’t possible — for example, due to archived backups — we will securely store your data and restrict access until deletion can be completed.
Non-personal information may be retained in anonymized and aggregated form indefinitely. You may contact us at support@form.space at any time to request deletion of your information.
Data Security
We take your privacy seriously and have implemented reasonable physical, technical, and organizational safeguards designed to protect the personal information we collect from unauthorized access, disclosure, loss, or alteration. These measures are designed to secure your data both during transmission and while stored. However, no method of transferring or storing data over the internet is ever completely secure. While we do our best to protect your information, we cannot guarantee absolute security.
Our databases are located in the United States. By accessing FORM from outside the U.S., you consent to the transfer, processing, and storage of your data in the United States. You are responsible for accessing FORM in a secure environment and maintaining the confidentiality of your login credentials.
Your Privacy Rights
Depending on where you live — including certain U.S. states, the European Economic Area (EEA), the United Kingdom (UK), Switzerland, or Canada — you may have rights that give you greater control over your personal information. These may include the right to:

Children’s Privacy
FORM does not knowingly collect, use, or sell personal information from anyone under the age of 18, or under the minimum age required in your jurisdiction. We do not market to minors, and our Services are not intended for children.
If you are under 18 years old, please do not use the Services or submit any personal information through the platform. By using FORM, you confirm that you are at least 18 years old or have received permission from a parent or legal guardian to use the Services.
In line with applicable U.S. privacy laws, we do not sell personal information from individuals under 16 years of age without affirmative authorization ("opt-in"):
If a user is between 13 and 16, we require direct opt-in from the user.
If the user is under 13, we require opt-in from a parent or guardian.
Any user who previously opted in may withdraw consent at any time.

Withdrawing Consent
If we rely on your consent to process your personal information, you may withdraw it at any time by contacting us. This will not affect the lawfulness of processing before your withdrawal, nor will it affect processing that relies on legal bases other than consent.
Opting Out of Marketing Communications
You may unsubscribe from marketing or promotional messages at any time by clicking “unsubscribe” in an email or replying “STOP” to an SMS. After opting out, we may still send transactional or service-related messages necessary to administer your account or respond to support requests.
We do not share mobile numbers or SMS opt-in data with third parties or affiliates for marketing purposes. Sharing with service providers (such as those supporting customer service) is permitted solely for operational reasons.
Account Information
You can review, update, or delete the information in your account at any time by logging into your account settings.
If you choose to close your account, we will deactivate or delete your account and remove your information from our active databases. Please note, we may retain limited information as needed to prevent fraud, resolve disputes, troubleshoot issues, enforce our terms, or comply with legal obligations.
If you no longer wish to use the Services, you may also email us at support@form.space to request deletion of your account and associated data. Please note that we may retain certain information as required by law or for legitimate business purposes such as fraud prevention, dispute resolution, or compliance with legal obligations.
Do-Not-Track Preferences
Most browsers and some mobile devices offer a “Do-Not-Track” (“DNT”) setting to signal your preference not to be tracked across websites.
At this time, there is no industry-standard protocol for how websites should respond to DNT signals. As a result, FORM does not currently respond to DNT requests or similar automated privacy signals.
If a standard emerges that we are required to follow, we will update this Privacy Policy accordingly.
United States Residents: Your Privacy Rights
If you are a resident of California, Colorado, Connecticut, Delaware, Florida, Indiana, Iowa, Kentucky, Maryland, Minnesota, Montana, Nebraska, New Hampshire, New Jersey, Oregon, Rhode Island, Tennessee, Texas, Utah, or Virginia, you may have certain rights under your state’s privacy laws. These may include the right to request access to the personal information we hold about you, understand how we use and share it, correct inaccuracies, delete your information, or withdraw consent where applicable. Over the past twelve (12) months, we have collected the following categories of personal information: identifiers (such as your name, contact information, account details, and device identifiers); personal information defined under the California Customer Records statute (including financial and employment-related information you may provide); protected classification characteristics (such as gender or age); commercial information (including purchase history and payment details); and general geolocation data.
We do not collect biometric data, internet browsing history, audio or video recordings, professional or education-related records, inferences drawn from personal data, or sensitive personal information as defined under applicable state laws.
We may also collect other information you choose to provide to us in the context of using our Services — including when you contact customer support, participate in surveys, or engage in community features.
We retain personal information only as long as needed to provide our Services or meet our legal obligations. In general, data from the categories listed above is retained for the duration of your account’s active use with FORM. To reduce data sharing, we recommend logging out of those accounts before using FORM.
Your interactions with these tools are governed by the privacy policies of the respective platforms. FORM does not control how that data is collected or used once transferred.
We may share personal information with trusted third-party service providers who assist in delivering the FORM experience. This includes providers supporting payment processing, customer service, marketing, technical infrastructure, and product development. These providers operate under written contracts that limit how they may use your information.
In the past twelve (12) months, we have not sold or shared personal information for monetary gain or for advertising purposes as defined by applicable privacy laws. However, we have disclosed certain categories of personal information to our service providers and partners for legitimate business or operational reasons. These categories include:

United States Residents:How to Exercise Your Rights

You may also authorize someone else to act on your behalf. If you do, we may request proof that they have been validly authorized to make the request in accordance with applicable state law.
Before fulfilling a request, we will need to verify your identity to ensure we are processing the request for the correct individual. If we cannot verify your identity based on the information we have, we may ask you to provide additional details for security or fraud prevention purposes.
If the request is made through an authorized agent, we may require signed written permission from you, along with separate verification of your identity.
United States Residents: Appeals
If we decline to take action on your request, you may appeal the decision by emailing support@form.space. We will respond in writing with the outcome of your appeal and provide an explanation. If your appeal is denied, you may have the right to contact your state’s attorney general to file a complaint.
California Shine the Light
California residents have the right to request, once per year and free of charge, information about the categories of personal information we may have disclosed to third parties for their direct marketing purposes in the prior calendar year, along with the names and addresses of those third parties.
